Deed Poll for Minors (13 or Under)

If you are the parent of a child aged 13 or under and live permanently in the Republic of Ireland, you can enrol a change of name by Deed Poll for your child.

One parent completes the Deed Poll form and the second parent signs their consent to the change of name. In some cases, a court order may be required if consent from either parent is not available.

  1. Complete the Deed Poll form

    One parent fills out the Deed Poll form. The second parent signs their consent. This must be done in front of a witness who is at least 18 years old.

    Enter the child’s old name exactly as it appears on the birth certificate, including all middle names.

    If the child has changed name before, you can use this as your old name and provide proof. For example, a previous Deed Poll.

  2. Get your documents certified

    Make photocopies of:

    • your child’s birth certificate
    • photo ID for the parent who completed the Deed Poll form
    • any supporting documents. For example, a parent’s death certificate

    Uncertified copies will not be accepted.

    You will need to have your document copies certified by one of the following:

    • An Garda Síochána
    • a practising solicitor
    • a Commissioner for Oaths
  3. Complete the affidavits

    The witness who saw the Deed Poll being signed must swear an Affidavit of Attesting Witness.

    The parent providing photo ID for the application must also swear a Supplemental Affidavit.

    The affidavits must be sworn in front of either:

    • a practising solicitor
    • or a Commissioner for Oaths

  4. Pay the fees

    Stamp duty is a government tax on legal documents. The documents must be stamped as follows:

    • Change of Name by Deed Poll document: €60
    • Supplemental Affidavit: €20

  5. Lodge your documents

    Please note: Minor applications cannot be sent by post unless a solicitor’s cover letter is enclosed. Without this, the application will not be processed and will be returned to you.
