Commonly used forms

Divorce – court forms
Divorce forms are used at different stages of the application process — for applying, serving documents on your spouse, replying to an application, and getting a court date. The forms you’ll use depend on whether you apply through the Courts Portal (online) or by submitting paper forms to a Circuit Court office.
