Court forms

Commonly used forms

These are the forms most often required in a divorce case. For full guidance on when each form is used, see the Divorce section of Family Law, or the step-by-step guide on how to apply for a divorce. Always check you are using the current version before filing. Where a form opens on the older website, it’s noted as “(legacy page)”.

Divorce – court forms

Divorce forms are used at different stages of the application process — for applying, serving documents on your spouse, replying to an application, and getting a court date. The forms you’ll use depend on whether you apply through the Courts Portal (online) or by submitting paper forms to a Circuit Court office. Each form is explained in context in our step-by-step guide.

For full guidance, see the Divorce section of Family Law.
